Document Requirements
Passport
The passport must remain valid at least 6 months
after the last day of the visa validity and should not cause any doubts about its authenticity and that it belongs to its owner. It should not contain any marks, erasures, corrections or any other records which are not certified by the competent authorities of a foreign state and should not have any torn pages. It should contain at least two blank pages.
Photograph
The size of the COLOR photo should be 35 x 45 mm and it should be printed on a good photo paper with a clear front of facial image. The photo must be taken within the last 6 months before the date of application and must show the full front view of the head, with the face in the middle of the photo, and include the top of the shoulders. The background should be plain white or light-colored.
Requirements for the travel insurance certificate
When applying for a Russian visa you should provide an ORIGINAL of a health insurance certificate or a clear letter of confirmation from your home insurance company containing the following information:
1) Name of the insurance company.
2) Policy number.
3) The word Russia (or the world).
4) Name of the applicant.
5) The period of insurance (must not be less than the period of the visa validity).
The document must be in the original form of the insurance company with the authorized signature and seal (stamp).
In the absence of this information, you should contact your insurance company and get the correct document. The majority of Swedish insurance companies are well aware of the consulate requirements to the insurance certificate for a visa to Russia. Therefore, in most cases, you just tell the insurance company that you need a certificate of insurance to get a visa to Russia, often called "Rysslandsityg".

Form (Application) must be filled in online via https://visa.kdmid.ru
Form must be signed by a blue ballpoint pen (signatures which had been made by ink or black color are not acceptable).

You must provide proof (an official certificate or a letter in any form). The Certificate "Справка о выходе из гражданства".
Or a detailed hand- written note with your signature and date containing your last full postal address in Russia/ USSR as well as when and where to you have immigtated.
